error when using image type inside form

Hi,

I tried to add image type inside form.

And javascript error occured when initialize the form.

Uncaught TypeError: Cannot set property ‘onchange’ of null

Can anyone help me? Thank you.

Could you please provide us complete demo including all correspondnig js/css files?
Please also add information regarding current and expected behaviour.
