Form untidy on layout

Hi all…
i have a form and put the form on the layout. but the form looks untidy at the bottom. The form should be like in the upload_works.php file. open file.php → click new toolbar

Please help me to fix this. Thanks

download tatest version from or site (it was released yesturday) seems like your issue already fixed.
