when I refresh the brower,uploaded file name disappeared

Hi Sir,

I saw the topic of SOS in dhtmlxForm and download the sample.

[url]SOS - Form - DHTMLX

That is Good sample with me. But I want to upload the img. It will display the file name in the upload box. Just like uploadSuccess.png.
But when I refresh the brower. The upload file name is disappeared. How can I make the file name appeare in the upload box that is alread uploaded.
p.s. : The file name and the path can save in the DB.

Could you give me ideas how can do it.
